Where each one earns its place
<p>The decision is not guided-versus-freehand as a philosophy. It is case-by-case. The questions that actually matter: how many implants, what is the prosthetic plan, and how much bone is there to work with.</p><p>Pick CBCT-guided placement when the case is full-arch or All-on-X, when immediate load is on the table, or when the implant sits in the esthetic zone and the emergence profile has to be right the first time. Choose CBCT-guided placement when the prosthetic is being designed before the surgery, because the guide and the provisional come off the same planning file and the lab can deliver a same-day temporary.</p><p>Go with freehand when the case is a single posterior implant in a healed ridge with generous bone volume, the patient has had implants before without complications, and the final crown is a standard screw-retained or cement-retained restoration with no esthetic constraint. Opt for freehand when the cost of the guide and the CBCT shifts the case out of the patient's budget and the clinical risk is genuinely low.</p>

For full-arch and All-on-X cases that is the entire pitch: one lab, one planning file, one accountability line. For single posterior implants in healed ridges, guided placement is often over-engineered, and a surgeon who prefers freehand on those cases is making a defensible call. We will still fabricate the final restoration either way.</p><p>Where we push back: esthetic-zone single implants. The emergence profile and the abutment angulation are too consequential to leave to intraoperative judgment when a CBCT and a guide cost less than a remake.</p>
