Where each one earns its place
<p>The decision sits on cusp integrity. If you scan a tooth with a failed MOD amalgam and the cusps still have bulk dentin support, an inlay restores the box without touching healthy enamel. If the same tooth shows a cracked lingual cusp or post-endo cusp flex, that cusp needs coverage and the case becomes an onlay.</p><p><strong>Pick an inlay when</strong> the prep is contained within the cusps, the remaining cusp walls are at least 2mm thick, and there is no visible craze line running down a cusp. <strong>Choose an onlay when</strong> at least one cusp is cracked, undermined, or sits on an endodontically treated posterior tooth that needs cuspal protection but does not need 360-degree coverage. <strong>Go with a full crown</strong> when two or more cusps are gone and the remaining tooth structure cannot support a bonded restoration.</p><p>For referring dentists sending us digital scans: mark the cusp coverage intent in the Rx. If you are unsure whether a borderline case is an inlay or onlay, send the scan with a note and the technician on the case will call back the same day.</p>
<p>Dani Dental mills both inlays and onlays from the same digital workflow. Lithium disilicate (e.max) is our default for anterior-visible premolars and esthetic posterior cases. Monolithic zirconia is the call for second molars under heavy occlusion or bruxism patients. Gold onlays are still available for dentists who prefer them on second molars; we keep the capability in-house because some clinicians correctly prefer gold's wear compatibility against opposing enamel.</p><p>Honest note: if the case is a single small Class I or occlusal-only cavity with no proximal involvement, a direct composite placed chairside is faster and cheaper for your patient than sending us a scan for an inlay. We are the right call when the prep crosses the marginal ridge, when esthetics matter, or when the cusp coverage decision needs a lab-fabricated restoration to do it right.</p>
