Locator retention vs bar overdenture for implant-retained dentures

<p>The decision is rarely about which attachment is better in the abstract. It is about ridge anatomy, implant count, parafunction, and what the patient can actually maintain at home. A 72-year-old with two healed mandibular implants, adequate ridge, and decent dexterity is a different case than a 58-year-old bruxer with a flat maxilla and four implants planned.</p><p>Pick locator retention when the case is a 2-implant mandibular overdenture with adequate residual ridge and a patient who needs straightforward home hygiene. Choose a bar overdenture when the case has 4+ implants, a resorbed ridge that cannot tolerate tissue-borne load, or a bruxer who will overload individual attachments. Go with locators when implants are divergent within ~20 degrees and the budget is tight. Opt for a milled bar when you need to correct path-of-insertion problems or distribute load across all implants in a parafunctional patient.</p><p>The maxilla is its own conversation. Locators on 4 maxillary implants can work, but a bar gives you the cross-arch rigidity most maxillary cases benefit from. Send the CBCT and intraoral scan to the lab before you commit to attachment style.</p>

If you are restoring a bruxer or a flat ridge with 4+ implants, the bar earns its cost in prosthetic longevity. We will not upsell a bar into a case that does not need one.</p>
