<p>The decision sits with the surgeon and the CBCT, but the restorative consequences travel downstream to the lab. Both diameter classes have defensible indications. The wrong choice for the site shows up six months later as a fractured implant body, a compromised emergence profile, or a crown that never looked right to begin with.</p><p>Choose standard diameter implants when the site is a posterior molar or premolar carrying full occlusal load, when buccolingual bone width measures 6.5mm or greater, and when the patient has any parafunctional history. Pick narrow diameter implants when the site is a mandibular incisor or maxillary lateral with limited mesiodistal space, when ridge width is 4.5mm to 6mm and grafting is contraindicated by patient health or timeline, or when the case is overdenture retention with two to four implants splinted by the prosthesis.</p><p>Go with a custom abutment on every narrow diameter case. The emergence profile margin is too tight for stock components to deliver the contour the soft tissue needs.</p>
