<p>The decision is driven by implant angulation, not preference. Once the surgeon places the fixture, the access channel trajectory is fixed; the restorative dentist either works with it or compensates with cement. The clean rule: if the screw access channel exits through the lingual or occlusal third of the planned crown, go with screw-retained. If the channel would breach the facial surface of an anterior tooth, opt for cement-retained on a custom abutment.</p><p>Choose screw-retained when the case is a posterior molar, premolar, or any anterior where the surgeon placed the implant palatally enough that the access hole exits behind the incisal edge. Retrievability matters more than most dentists weigh it at delivery; five years later, when the porcelain chips or the patient needs hygiene access, sectioning a cemented crown costs an hour of chair time. Pick cement-retained when angulation forces a facial access hole on tooth #8 or #9, or when interocclusal space exceeds 5 mm and the case calls for an angulated custom abutment with a screw-retained crown cemented extraorally (the hybrid approach, which captures both benefits).</p><p>For ambiguous cases, send the CBCT and intraoral scan to the lab during planning, not after placement.
