The metal alloys used in porcelain-fused-to-metal crowns, typically high-noble gold alloys or base-metal alloys such as nickel-chromium or cobalt-chromium, have a multi-decade safety record backed by FDA clearance and ADA acceptance. Documented allergic reactions to dental alloys occur in fewer than 1% of patients. For the rare nickel-sensitive patient, full-zirconia or all-ceramic alternatives eliminate metal contact entirely.

What sits behind it

PFM crowns use a cast metal substructure, typically high-noble (gold-palladium), noble (palladium-silver), or base-metal (nickel-chromium, cobalt-chromium) alloy, veneered with feldspathic or pressed porcelain. High-noble alloys carry the strongest biocompatibility track record and minimal sensitization risk. Base-metal alloys are more affordable and still ADA-accepted, though nickel-containing alloys carry a small documented sensitization rate, most often relevant in patients with existing contact dermatitis.

For general dentists and prosthodontists selecting a crown material, the clinical decision is rarely about alloy safety in isolation. It comes down to occlusal load, esthetic zone placement, prep reduction available, and patient medical history. Posterior teeth under heavy bruxing load or patients with limited prep depth are still strong PFM candidates, precisely because the metal substructure resists fracture in a way that full-ceramic units cannot always match.
