Is zirconia safe?

Zirconia became a mainstream restorative material in dentistry around 2005, replacing porcelain-fused-to-metal in many applications precisely because its biological profile is cleaner. The ceramic matrix is chemically inert at body temperature. Unlike base-metal alloys, it does not oxidize or shed ions into the sulcular fluid. Long-term clinical data from orthopedic femoral heads, where zirconia has been load-bearing in the human body for 30-plus years, provides the strongest biocompatibility evidence base.

For the referring dentist, the relevant safety question is almost always framed around two patient presentations: documented nickel or metal allergy, and patients requesting metal-free workflows for personal or systemic-health reasons. Zirconia satisfies both. Monolithic high-translucency zirconia restorations eliminate the porcelain chipping risk of older PFM designs and remove every metallic component from the crown or bridge unit. That matters for tissue response at the margin, particularly on implant cases where emergence profile and peri-implant health are under the microscope at every recall.
