Who is a good candidate for All-on-X?

THE SHORT ANSWER
Adults with most or all teeth missing in one or both arches are the core candidates for All-on-X, provided they have adequate bone volume at the implant sites or can receive bone grafting. Medically stable patients with controlled diabetes or periodontal disease also qualify after clearance. Edentulous patients tired of ill-fitting dentures are the most common referral profile.

What sits behind it
The classic All-on-X candidate presents with advanced tooth loss, failing dentition, or full edentulism in at least one arch. Bone volume at the anterior ridge is the primary surgical gate. Posterior atrophy is common in long-term denture wearers, which is why tilted implant placement was developed: angling the posterior fixtures lets surgeons avoid the sinus and maximize contact with available bone without mandatory grafting.
Medical status matters more than age. A 45-year-old with uncontrolled type 2 diabetes is a harder candidate than a healthy 72-year-old. Patients on bisphosphonates or with significant immune suppression require individual case review. Oral surgeons and prosthodontists typically coordinate this screening before the restorative team engages the lab for a provisional plan.
